At Five Points Medical Center, we want to walk beside you on your journey to wellness. That journey looks different for each or us and there are many factors that affect wellness. Obesity is one of the factors that has a significant effect on our health and wellness and can lead to many chronic diseases. We believe that chronic disease can be prevented and reversed by healthy living including weight loss, good nutrition habits, exercise, and stress reduction. It would be our privilege to guide and coach you on your wellness journey.
Below is an overview of our services and costs. Please note that we do not accept commercial insurance for our services. The costs listed below are cash prices. All payments are due at the time of service.
Medical Weight Loss and Wellness Services
First Visit
One Hour, In-person
$250
- Vital signs
- Measurements: arms, chest, waist, hips
- Labs/testing
- Completion of medical history and weight loss questionnaires
- Discussion of weight loss options
Patient will be advised to call their medical insurance company to see if any weight loss medications are covered between their first visit and second virtual visit.
Second Visit
30 minutes, Virtual
Cost included in first visit
- Discussion of lab results
- Nutritional needs assessment results discussed
- Nutrition/Exercise/Calorie/Macronutrient information shared and packet emailed to patient
- Weight loss medication prescribed as appropriate based on labs/insurance/weight loss goals
Follow-up visits
In-person monthly
$100/month (if getting medication through local pharmacy)
$400/month Compounded Semaglutide
$450/month Compounded Tirzepatide
- Vital signs
- Measurements
- Lipase lab if using Semaglutide or Tirzepatide at 1month f/u and 3 month f/u
- Adjust prescription dosing if needed
*After 3 months on medication, abnormal labs from initial visit will be repeated and discussed virtually with patient once reviewed
*Lipotropic injection for energy boost and fat lipolysis available at $10 per injection 1-2 times/week if appropriate
